Or something like that.What should i write?
Tell them what qualities you have, what you can bring to the workplace. examples: cheerful, offer creative solutions, good with children, honest, dedicated worker, team player, lively work environment is invigorating. i don't think it really matters.

if it's anything like the chuck e cheese i work at you'll get an interview whether you fill it out or not.

i put down something like, I'm very nice and smiley. I get along well with everyone. I'm great with kids and people of all ages. And i liked going there when i was a kid and think i would enjoy having a job there.
